Who You Are

  • You are a qualified professional with an occupational therapy qualification recognized by the HCPC and have current HCPC registration.
  • You possess a strong understanding of the legal social care framework and relevant policies, with specific expertise in the Care Act, Mental Capacity Act, Mental Health Act, and safeguarding practices.
  • You have experience in completing holistic assessments and reviews, working with community settings, and undertaking Best Interest and mental capacity assessments.
  • You are proficient with IT systems and have experience working within multi-disciplinary teams.
  • Your approach is strengths-based, person-centered, and community-focused, allowing you to build effective relationships and achieve positive outcomes for people and carers.
  • You are a persuasive communicator, an effective planner, dedicated to achieving excellent customer service, and uphold high personal and professional standards.
  • You respect diversity and demonstrate integrity, acting as a role model for appropriate behavior while recognizing and valuing the contributions of others.
  • You are committed to your own continuous professional development and willing to share knowledge and mentor others.

What the Job Involves

  • Provide effective adult social care services by conducting high-quality assessments and developing person-centered, asset-based interventions.
  • Manage caseloads depending on your experience and the complexity of cases, using specialist knowledge for interventions, care planning, risk assessments, and safeguarding investigations.
  • Work collaboratively with health services to ensure required health and social care support is available in the community, facilitating timely hospital discharges and admission avoidance.
  • Explore support options and resources, including personal budgets, to maintain individuals' independence in their homes and communities.
  • Ensure thorough and accurate case recordings, and prepare comprehensive reports, complying with data protection and GDPR requirements.
  • Participate in multi-agency frameworks, conduct Liberty Protection Safeguard assessments, and manage resources effectively to maintain independence for service users.
  • Develop and implement practice improvements, maintain quality standards, and undertake new roles in line with legislative developments.
  • Liaise effectively across agency boundaries to ensure integrated care and support, including attending and providing expertise at various meetings and conferences.
  • Supervise students on professional placements and manage non-qualified staff, including apprentices, while ensuring their development through appropriate training.
  • Handle safeguarding issues according to policies, ensuring the protection of vulnerable adults and complying with relevant legislation.
