What the Job Involves

As an Agency Social Care Assessor working within the Adult Access Team at the Cunard Building, you'll provide secondment cover and contribute significantly to the team. Your duties will include conducting assessments and providing reablement support, as well as assessing eligibility for ongoing community support. You will work diligently to prevent, reduce, and delay the need for care and support across all interventions. The role involves managing a varied caseload, addressing issues such as hoarding, substance misuse, eldercare, mental health, and learning disabilities, among others.
