What the Job Involves
This role requires you to be part of a peripatetic team providing short-term support services across various regions, including Fylde and Wyre, Lancaster and Morecambe, Preston and South Ribble, Skelmersdale and Ormskirk, and East Lancashire. Your responsibilities involve triaging cases before hospital discharge, conducting reviews, including completing DSTs promptly post-discharge, and managing urgent tasks while on duty, ensuring seamless support for individuals leaving hospital care.
