JOB DESCRIPTION & PERSON SPECIFICATION Job Title: LD and Autism Keyworker Band: NHS Band 6 WTE (£40,701- £48,054) Department: Barnet, Enfield, and Haringey CAMHS Location and mobility: Your normal place of work will be at St Anns Hospital Haringey. However, you may be required to work at any other location of the Trusts interests, including travelling to appropriate meetings, outside of the borough, as required. The aim is to keep children/young people in the community, to prevent avoidable admissions to Tier 4 inpatient mental health services and 52-week residential placements, and to support young people in this cohort to be discharged from Tier 4 services and be supported back into the community. You will provide high quality support to the children and young people and their families/carers, working as part of a team to help unblock the system, provide a bridge across different parts of the system including health, education, and social care, and be a key point of contact for families/carers who are part of the LD & Autism Programme. In addition, you will work with other professionals across the system to directly support families/carers across various settings to support the young person to remain in the community. Job Summary: The Keyworker is part of the NHS long term plan. The vision is for all children and young people with learning disabilities and/or autism to have a designated key worker by end 2023/24. Initially the priority will be for all children and young people currently in hospital to have a key worker, then for children and young people who are at risk of going into hospital (red/amber on the Dynamic Support Registers), and with evaluation of the pilot, the aim will be to develop a Keyworking service for all children and young people with learning disabilities and/or autism by end 2023/24. The 0-18 years Keyworker model aims to support reduction of the number of children in crisis and Tier 4 admissions, support timely assessments/formulations, provide children and families with the support that they need at the right time, improve transitions and quality of life, resulting in happy children and parents. A key function will be to empower families and support them with navigating barriers across multiple services and organisations to effectively access the right support from the system. Our Keyworking model is about a set of functions introduced to the system via the Keyworking role, in addition to the current roles in place to support children, young people and their families. The Keyworking function will dynamically connect the system, coordinating care systemically to maximise local resources and work more effectively with families. Each CYP will have a dedicated keyworker as their navigator, however they can access support from any co-ordinator at any one given time to link a Keyworkers particular area of expertise with CYP needs. We recognise that our keyworkers need to be experienced to be effective in this pivotal role. We will recruit Keyworkers across social care, NHS, voluntary sectors, and experts by experience, making allowance for Local Authority and NHS working terms and conditions to be continued where applicable. The Keyworker will join a Professional Lead Keyworker, will work outside of the constraints of individual services (such as eligibility criteria), and have access to the CYPs records across Social Care, Health, and Education, to achieve positive outcomes for children, young people, and their families. PERSON SPECIFICATION GENERAL INFORMATION Job Title: LD and Autism Key Worker Band: NHS Band 6 WTE Permanent Post Department: Barnet, Enfield, and Haringey CAMHS EDUCATION and EXPERIENCE A/I E1 Minimum Band 6 level (or equivalent), such as Learning Disability Nurse (RNLD), Mental Health Nurse (RMN), Psychological Wellbeing Practitioner (PWP), Mental Health Social Worker (SW), Occupational Therapist (OT), Speech & Language Therapist (SLT). OR Experienced Assistant Psychologists, Therapeutic/Community Workers or SEN staff, with a professional qualification in health, social care or education to level NVQ / HNC level 3 (or equivalent) or the ability to complete a relevant qualification evidenced by educational history and professional development. A E2 Experience of direct work with children and young people, or adults, in the context of learning disabilities and/or autism who are in crisis, with associated health, education and care needs. A/I KNOWLEDGE, SKILLS and ABILITY E3 The ability to work flexibly and responsively to provide support when children, young people, families/carers need it most, which may include early morning, evenings and weekends as required. A/I E4 Some knowledge of relevant legislation, guidance, government policy and research in relation to children and young people with learning disabilities and/or autism and an ability to apply that knowledge in an operational setting. A/I E5 An awareness of the risks associated with neurodevelopmental differences and social, emotional and mental health needs, with the ability to use this knowledge to safeguard the children and young people in your care; to promote and safeguard the welfare of children and young persons in your care, respond to and report any concerns about the welfare of a child promptly in line with local guidance on safeguarding children. A/I E6 The ability to provide structured support, deliver positive activities and ensure the achievement of agreed individual outcomes. A/I E7 Ability to work therapeutically within and contribute to multi-agency risk assessments for children and young people with complex needs, including risks associated with behaviours of concern and support for children and young people with complex mental health needs. A/I E8 Ability to support positive, planned risk-taking that promotes developmental growth and children and young peoples rights to community access. A/I E9 Ability to deliver good outcomes for children and young people and their families/carers, working within planned interventions agreed by the professional network. A/I E10 Ability to form and maintain appropriate professional relationships and boundaries with children, young people and families, carers, colleagues and the multi professional network. A/I E11 Knowledge of child development, autism, learning disabilities, attachment and trauma, and of personalised and coproduced care planning. A/I E12 Ability to creatively engage, communicate and consult with children and young people with learning disabilities and/or autism and their families/carers about developing and improving their service. A/I E13 Ability to communicate clearly, effectively and concisely both verbally and in writing and to promote a positive workplace culture that puts the children and young peoples needs at the centre of everything we do. A/I E14 Ability to manage your time effectively, adding value to the service where possible and using IT systems to maximise your efficiency.
